3. Financial Responsibility
By receiving services from our practice, you agree to the following:
You are financially responsible for services rendered, whether covered by insurance or not. Estimates provided are not a guarantee of insurance reimbursement.
A 72-hour cancellation/refund policy applies to all prepaid discounted services per Florida law (F.S. §456.062).

We are committed to protecting your Protected Health Information (PHI) in accordance with the Health Insurance Portability and Accountability Act (HIPAA).
